Consent Form for Accommodation of a Minor

■ Information from the Hotel (Important Notes)

1. One copy of this consent form is required for each guest.

2. When submitting this consent form, please be sure to also submit a copy of one form of identification for the person with parental authority who completed it (one of the following: driver’s license, passport, residence card, or My Number Card (front side only)), together with this form (e.g., a photo of the document).

3. Please also be sure to submit a copy of identification for the guest (one of the following: student ID, health insurance card (including certificate of insured status), passport, residence card, or My Number Card (front side only)). If someone other than the person with parental authority is accompanying the guest, please also submit a copy of identification for that companion (one of the following: driver’s license, passport, residence card, or My Number Card (front side only)), together with the above.

4. Please email this consent form and copies of the identification documents to the operating company below no later than 7 days before your check-in date.

5. To confirm your reservation, we may telephone the person with parental authority at the emergency contact number provided.

6. If we are unable to confirm the above documents (this consent form and identification for the person with parental authority, the guest, and any companion) by the deadline, we will decline the stay (cancel the reservation) in accordance with the Accommodation Terms and Conditions (Article 6, Item 11, or Article 9, Item 17). In addition, if we are unable to reach the person with parental authority by telephone at the emergency contact number when confirming the reservation, we will treat the above identity-verification process as incomplete and handle it in the same way. In this case, in accordance with Article 9, Paragraph 2 of the Accommodation Terms and Conditions, we will not charge for any accommodation services not yet provided. Furthermore, because this constitutes cancellation of the contract by the hotel rather than a cancellation for the guest’s convenience, no cancellation fee under Appended Table 2 will apply.

7. If, during the stay, the guest engages in conduct that disturbs other guests, damages hotel facilities, or violates smoking rules within the building or on the premises, we will contact the person with parental authority immediately, and the person with parental authority will bear the full cost of any resulting liability (including restoration costs and other damages).

8. If forgery or false statements are discovered in this consent form or the identification documents, we may take legal action, including reporting the matter to the police.

9. This consent form and the identification documents will be used solely for the purposes of confirming the guest’s identity, making emergency contact, and responding to matters such as claims for damages, and will be properly disposed of in accordance with the retention period set out in our Personal Information Protection Policy (Privacy Policy) (for information subject to a retention period under the Hotel Business Act or other laws, that period; otherwise, a reasonable period after the purpose of use has been achieved).

For both the person with parental authority and the guest, if you submit a copy of your My Number Card, please be sure to photograph and send only the "front side (the side with the photo)."

Please never send an image of the back side (the side containing the individual number), as this is restricted by law.
